This page gives some tips on how to swap the Command key with Option key on Mac OS X. (or switching any modifier keys)

Starting with Mac OS X version 10.4, you can switch modifier keys to any other. This is done in System Preferences → Keyboard & Mouse.
If you are using older version of OS X, you might look at http://gnufoo.org/ucontrol/ucontrol.html. Or, if you want more versatile key-remapping for OS X, see: fKeys↗ and doublecommand↗. Also, if you buy a Microsoft keyboard, it comes with a software called IntelliType that allows you to swap modifier keys. Another solution is via the commercial software QuicKeys↗, available for both Mac and Windows. QuicKeys is the most robust keyboard macro software i've used from ~1992 to 2002. It now has both Mac and Windows versions.
For Windows or Linux, there are several sites on the web that tells you how. Search the web for “swap Control Alt keys”.
For Linux users, GNOME↗ and KDE↗ both provides GUI for switching to the Dvorak layout and swapping modifier keys. If all fails, you can resort to the xmodmap utility (see “man xmodmap”). Here's a xmodmap file for dvorak dvorakKeymap.txt.
